I have a small problem and I started ...
In C: \ Documents and Settings \ All Users \ Start Menu \ Programs \ WinMerge
In the properties of readme, I have this:

in place of %systemroot%\system32\shell32.dll
So how to do this step:

[readme.AddShorcut] Name = Read me CmdLine = 16422, WinMerge, ReadMe-english.txt Subdir = WinMerge WorkingDir = 16422, WinMerge IconPath = 11,,notepad.exe
No need for IconIndex because by default it will use the 1st icon in the DLL or EXE
Why dont you like the icon in shell32?
If you would like to use that one here is that code
[readme.AddShorcut] Name = Read me CmdLine = 16422, WinMerge, ReadMe-english.txt Subdir = WinMerge WorkingDir = 16422, WinMerge IconPath = 11,,shell32.dll IconIndex = 70
