I thought it's designed to pickup \boot\etfsboot.com in the iso folder, not from System32

It's not something official or solid
and not All LDR branches are dead
but all released Windows 8.1 updates and hotfixes since RTM, all are GDR only
except some .NET hotfixes and security updates, some was LDR only, and one of them is GDR+LDR (KB2898871) -
It's just a regular MSU package that can be integrated normally
LDR branch is dead in Win8.1 (except .net hotfixes)

Did you integrate them in Normal mode or LDR mode?
also, did you integrate them before or after Platform Update KB2670838?
because those two are bi-updates, applicable with or without KB2670838
meaning they contains component for normal system (without KB2670838), and component for KB2670838

Are the files in the first post updated to include two latest updates?
KB2898871 & KB2901128
Those are Windows 8.1 updates
nevertheless, the addon contain lateset hotfixes (including security updates that are released in February

The 1 file IE prerequisites you get is a simple text file, you need to open it manually and make sure you integrate the updates mentioned

.NET 3.5 Family and .NET 4.x Family are completely separate software package
they can co-exist together, but 4.x does not replace or has effect on 3.5
so the updates you are getting has nothing to do with this Addon
and you need to install those updates

Yes, it's updated with same rollup
the Regular Addon is technically the same thing, it's eliminate "netfx_Full_LDR.mzz" file which is around 200 MB + Windows6.1-KB958488-v6001-*.msu which is +4MB (x86), -7MB (x64)

IE10 Updates has IE10- prefix in ther names, whereas IE11 Updates has IE11- prefix
so what is the same between them exactly?
and about the folders, IE10 and IE11 are unique to each other, so users who wants IE11 will not download any IE10 related, and vice versa

I'm a bit confused, after download latest and delete supersedes for all folders:

There is nothing wrong (explicitly )
IE Prerequisites are clear enough (i'm thinking of changing the way for providing them to a single text file, this way we can get rid of the duplication situation)
AD LDS & RSAT duplicates are necessary to have them in both locations, because each of them contain components that are applicable for both packages
same for RDC8.1 and RDP8.0
this also includes KB2834140, it's bi-update (meaning its components are applicable with and without Platform KB2670838)
IE10 CABs:
KB2764913 is the same IE-Hyphenation-en, i changed the name to KB2764913 to avoid duplication with IE11 Hyphenation
same thing goes for KB2764916 and IE-Spelling-en
KB2764916 + KB2764913 are neutral packages (meaning x86 and x64 are the same file)
so, you need to delete IE-Hyphenation-en + IE-Spelling-en from IE10 CABs folder
KB2892074 and KB2893294 and KB2900635:
apparently those duplication is a temporary error or something caused by WHD
you can safely delete Windows8.1-KB2900635-x86 from x64 folder
and redownload KB2892074, KB2893294 for windows8.1

How did you got 523 hotfixes? there are curently about 330 only
and all updates in the list are total 589 (with duplicates)
the duplicates are about 29
